We are pleased to offer an enhanced pharmacist-led service across the Runcorn GP practices, delivered by a dedicated team of Clinical Pharmacists and Pharmacy Technicians employed by GP Health Connect Ltd.

This service is designed to support patients with their medicines and includes:

• Structured Medication Reviews (SMRs): In-depth reviews to ensure patients are on the right medications, they are taken safely and effectively and tailored to their specific needs—particularly for those with long-term conditions, on multiple medicines, or at risk of medication-related problems.
• Medicines Optimisation: Ongoing work to ensure medicines are used in the most effective and evidence-based way, supporting better health outcomes and value for patients and the NHS.

Our pharmacists and pharmacy technicians are based within your GP practice and work closely with your healthcare team. If you feel you would benefit from a medication review or have any questions about your prescriptions, you can request an appointment by contacting your GP practice directly.
